Share your experience of health and social care in Conwy

The Community Services Transformation programme is a joint-partnership project and its purpose is to deliver on the vision of Welsh Government for sustainable and integrated Community Health and Social Care.

The ultimate aim is provide seamless services to the people of Conwy & Denbighshire.

Health and social care partners in Conwy and Denbighshire - Conwy County Borough Council, Denbighshire County Council, Betsi Cadwaladr University Health Board, Primary Care and the Voluntary and Community Sector want to hear about your experience of how health and social care services are being delivered in reality.

Working together, the partners want to ensure all residents get the best possible support to stay well and live healthier lives for longer.

To help understand how this can be achieved there will be twelve virtual workshops asking people for their views about how everyone are working together.

Each workshop will be via Zoom and will last for 90 minutes and you can register here.

10.30-12.00: Have you been supported by a Community Resource Team or you are the carer /relative of someone who has? We would like you to take part in this workshop to better understand your experience of how health and social care worked together. Community Resources Teams are made up of professionals from health and social care. This includes, social workers, nurses, occupational therapists, physiotherapists and voluntary organisations. The teams work closely with GPs, families and carers to help people with care and support needs to remain living at home.

10.30-12.00: Are you an older person living in a remote or rural area or are the carer /relative of someone who is? We would like to talk to you to understand your/their experience of getting support and/or services from health and social care.

2.00-3.30: If you have a respiratory condition or are the carer/relative for someone that does, we would like you to take part to better understand your/their experience of needing help and/or support from health and social care services. Chronic respiratory conditions affect the airways and other structures of the lung. Some of the most common are chronic obstructive pulmonary disease (COPD), asthma, occupational lung diseases and pulmonary hypertension.

10.30-12.00: Adults with a Learning Disability and Health Checks - we want to better understand your experience of how health and social care can make it easier for you to attend your annual health check.

2.00-3.30: If you have received end of life care before the pandemic either personally or as a witness, likely to be by carers and/or loved ones, we would like to encourage your participation. Palliative/End of Life Care often involves a number of different people working together, including the GP, community nurses, social care workers, care home staff or carers.Covid 19, which had a devastating impact on end-of-life care, will be looked at elsewhere so this will not be the central focus of the workshop.

10.30 - 12.00: Mental Health Challenges and support services – are you an adult (18+) with personal experience of mental health challenges/problems over the last three years or are you the carer/relative/friend for someone who has? We wouldlike you to talk with you to better understand your/their experience of needing help and/or support from health and social care services.

2.00-3.30: Care Leavers experience of Health and Social Care - have you left care in the last three years? If you have, we would like to talk to you to understand your experience since leaving care of services and/or support from health and social care.

10.30-12.00: Mental Health inpatient and transition to community-based support – have you been in hospital within the last three years because of your mental health? If you have and/or you are the carer /relative of someone who has, we would like to speak to you to understand your experience of how health and social care worked together following your discharge or transfer from an inpatient mental health setting to the community.

10.30-12.00: Are you a young person (18+) with ‘complex needs’ who has been through a planned transition (move) from health and social care services provided for children and young people to services provided for adults in the last three years, or are you the parent / guardian / carer / of someone who has? We would like to talk to you to better understand your experience of how health and social care worked together. People who have complex needs may have been diagnosed with an illness, disability or sensory impairment from birth or as a result of illness or injury and may need a lot of additional support on a daily basis.
