The Health and Safety Executive has issued an improvement notice to Conwy County Council after a fatal incident in Llandudno involving a lorry and a pedestrian.
Daphne Stallard, 89, of St Mary's Road, died in September after being struck by a reversing lorry.
The notice highlights the Council's failure to produce adequate risk assessments for trade waste routes and to ensure waste operatives are following safety measures.
The executive can issue such notices in cases of health and safety breaches and take action to ensure that safety procedures are implemented without fail.
